Two names you need to know:
Cigna Life Insurance New Zealand Limited (Cigna): Is your insurance company. This is Cigna’s research panel. This research community does not impact your Cigna insurance policy document in any way.
Camorra Research Limited: The research company Cigna have partnered with to manage the day to day running of the research panel. Camorra will be developing the surveys, looking at responses and providing Cigna with the results. When you receive emails from the community Camorra are sending these on Cigna’s behalf.
In these terms:
a) “Cigna” means Cigna Life Insurance New Zealand Limited
b) “you” means you, the person that has registered to participate in the Shaping Change research panel and surveys
c) “Shaping Change website” or “website” means this website; and
d) “surveys” or “research” means any research activities carried out by Shaping Change including surveys, online discussions, or focus groups.
Your contribution and feedback is very important because it will help Cigna to improve the products and services it offers to their customers and advisers. Your participation is at your discretion.
Application of these Terms and Conditions:
The Shaping Change Terms and Conditions only govern Shaping Change. They do not alter or affect your policy terms and conditions.
Consumer Guarantees Act 1993
Nothing in these terms and conditions is intended to exclude, restrict or modify a consumer's rights under the Consumer Guarantees Act 1993.
In order to qualify as a member of Shaping Change you must:
be a current customer of Cigna or be a current adviser for Cigna;
be at least 20 years old;
Not work for another insurance company or media outlet
register online at https://shapingchange.cigna.co.nz providing your full name, email address, and complete an introductory survey which we will use to determine your suitability for the Shaping Change research panel; and
be approved as a Shaping Change research member.
Ending your Shaping Change membership:
You may end your Shaping Change membership at any time by clicking on the “Unsubscribe” link at the bottom of your survey invites or by emailing your request for removal to firstname.lastname@example.org.
We reserve the right to suspend, restrict, or otherwise terminate your Shaping Change membership, at any time, without notice or liability to you. However, if we terminate/suspend/restrict your participation, this will not affect your potential entitlements to any prizes already drawn. Your participation in surveys, online discussions, and focus groups is entirely your choice. However, if:
you haven’t participated in a survey, online discussion, or focus group in over a year;
you're no longer a Cigna customer or adviser;
we can’t reach you by email following three hard bounces indicating an invalid email address or ten soft bounces indicating the email is undelivered
Then we may discontinue your Shaping Change membership without notifying you.
By ending your membership with Shaping Change, you will be removed from the Shaping Change mailing list and as such no longer receive emails about Shaping Change research opportunities. Ending with Shaping Change does not impact your Cigna insurance policy in any way.
Use of our Shaping Change website:
Cigna provides this Shaping Change research panel with reasonable care and skill. However, to the extent permitted by law, Cigna does not warrant or represent that this website will operate on a continuous or fault-free basis, or that it will be free of viruses or other harmful material.
The information provided on this website has been supplied by Cigna, in partnership with Camorra, unless otherwise indicated. Whilst all reasonable endeavours have been made to ensure that the information supplied is accurate and up to date, errors and omissions may occur.
To the extent permitted by law, Cigna does not accept any liability for any loss or damage which may directly or indirectly arise as a result of any advice, opinion, information, representation, error or omission, whether negligent or otherwise, contained on this website, or for any delay or interruption when using this Shaping Change website.
The Shaping Change website uses both persistent and session cookies. Cookies are small pieces of information, which can be stored on your hard drive (persistent cookies) or in memory (session cookies). The cookies we send to your PC cannot read your hard drive or command your computer to perform any action.
The session cookies hold information that is uniquely generated when you log in. They are used to determine that you are who you say you are, and to provide you with information whilst you are logged into the Shaping Change website. No personal information is kept in the cookie and the cookie is not permanently written to your hard drive. When you log out of the Shaping Change website the cookie is no longer valid and is discarded when you close your internet browser.
Participation in Shaping Change research:
As a member of Shaping Change, you may be contacted from time to time requesting you to take part in market research to get your views on a wide array of insurance and financial topics (e.g. insurance products, services, marketing and ideas), but your participation is voluntary.
The surveys you will be invited to participate in will be conducted by market research professionals, who have been engaged by Cigna. If you are invited to participate in a focus group, these will be conducted at a time and location advised in the invitation.
Registering and being approved as a Shaping Change research panel member does not guarantee that you will be invited to participate in any surveys. The persons selected to participate shall be at our sole discretion, based on the criteria required for each specific research project.
Any responses you provide will be aggregated with the responses of other participants. We will not disclose your individual responses in a manner that identifies you without your prior consent.
You have rights of access to and correction of your personal information. If you’d like to ask for access to your information, or to have it corrected, please contact email@example.com.
Privacy at membership end or non-qualification:
Once your Shaping Change membership has ended or in the event that you do not qualify to become or are not approved as a Shaping Change research member, any personal information you have provided (such as your name and email address) will not be further used by Cigna. Any aggregated information arising out of your participation in our research may still be kept, but neither you nor your personal information will be identifiable.
You must ensure that:
you will only register for Shaping Change once;
you do not use this Shaping Change website in breach of any legislation or for any unlawful act, fraudulent or improper purpose, or do anything to damage or disrupt this Shaping Change website;
all information you supply is true, complete, and accurate in all respects (and if any of your circumstances change resulting in any information you have previously provided no longer being true, complete, and accurate in all respects you will immediately notify us by email at firstname.lastname@example.org);
you keep all passwords, logins, usernames, and any other security information that we provide to you, or you otherwise use in connection with Shaping Change, confidential and you do not disclose the same to any third party (and if you know or suspect any other person has obtained your logins and usernames and any other security information, you must immediately notify us at email@example.com);
you do not introduce any harmful information or materials (which includes any spyware, viruses, worms, trojan horses, harmful scripts, codes, or any other damaging items which could interfere with this website, our software, hardware, network, or computer system).
All research is to be treated as confidential. You must not disclose, store, copy, or distribute any information about the Shaping Change research without our express permission in writing.
You must not participate in any of our research with a view to intentionally manipulating or detrimentally impacting the results of the research.
Copyright in this Website (including text, graphics, logos, icons, sound recordings, and software) and other intellectual property rights is owned or licensed by us. Everything on this Shaping Change website is copyrighted unless otherwise noted. You may not copy, modify, store, or display for redistribution to third parties or for commercial purposes any part of this Shaping Change website without Cigna’s express permission in writing. Non-commercial personal use of the Shaping Change website is welcomed, provided that you do not remove any copyright, trademark, or other intellectual property contained in the content. Without limiting the foregoing, you acknowledge that any and all research, data, and information provided by you will be our intellectual property.
Upgrades and maintenance:
This website may be unavailable from time to time due to upgrades being undertaken, errors, defects, maintenance, or events that may be outside of our control.
Any sites linked to this Shaping Change website are not under the control of Cigna. To the extent permitted by law, Cigna is not responsible for, and makes no representations or warranties concerning, the contents of any linked site or any link contained in a linked site. Cigna provides links to you as a convenience only and the inclusion of any link does not imply endorsement, investigation, or verification of the linked site by Cigna.
Please do not at any stage disclose any of your specific policy numbers, login or password details in surveys or in emails to firstname.lastname@example.org. Cigna Shaping Change will never request this information from you.
Changing these terms and conditions:
We may change these terms and conditions at any time by adding to, removing, or amending the existing terms. Amendments may take the form of completely new terms and conditions. We will inform you about any changes that we make by posting updated terms and conditions on the Shaping Change website. If the changes are material, we will contact you by email. If you do not agree to the updated terms and conditions, you will have the ability to terminate your membership.
You may not transfer or otherwise assign your Shaping Change membership or log in details to any other person.
These Terms and Conditions, and use of this website, is governed by and are to be interpreted in accordance with the laws of New Zealand.
PRIZE DRAW TERMS AND CONDITIONS
The prize draw is sponsored by Cigna and drawn monthly by Cigna’s research panel partner, Camorra.
One entry to the monthly prize draw is given to:
a. current Shaping Change members who have completed a survey in the month
b. new Shaping Change members who joined in the month.
Members who join and complete a survey in a month will receive two entries.
The monthly prize is one of six Prezzy® cards. Every calendar month Shaping Change is giving away one $500 Prezzy® card and five $100 Prezzy® cards. The first six entries drawn that comply with these terms and conditions will win the prizes. Prezzy® cards Terms and Conditions and can be found at www.prezzycard.co.nz. The winners of the prize will be required to comply with the Prezzy® card Terms and Conditions.
The monthly prize draw will be randomly chosen within the first 10 business days following the month end and the winners will be notified by email within the first 15 business days following the month end. Participants should ensure their contact details registered with Shaping Change are correct. If three reasonable attempts have been made to contact the winner by email and the winner is still unavailable, the prize will be forfeited and a new winner will be chosen.
From time-to-time additional prizes may be offered. The type and/or value of the additional prizes and how many members can win will be specified in eligible survey invitations. Winners for additional prizes must comply with these terms and conditions to win the prize.
Our decision is final and no correspondence will be entered into. The prize is not transferrable or redeemable for cash. If for any reason a prize becomes unavailable, Cigna may substitute an alternative equivalent prize without notice.
By accepting the prize, the prize winner agrees to their details being published by Shaping Change.
Employees of Cigna and its related companies, their spouse (either married or de facto) and their dependents (children and/or parents living with them) are not eligible to participate in the Shaping Change prize draw.
Cigna reserves the right to change, or terminate a prize draw at any time.
